Allegations (LADO) Referral Form (Word, 209KB) – to be completed by the senior manager (or other designated person who provides the information) of the employing agency for the adult concerned. Please note telephone contacts will not be accepted, the referral form should be completed and submitted to the secure LADO mailbox.

Disclosure and Barring Service

On 1 December 2012 the Independent Safeguarding Authority (ISA) and the Criminal Records Bureau (CRB) merged to form a new organisation, the Disclosure and Barring Service (DBS). The legal duty to make referrals remains, however referrals should now be addressed to the DBS. The DBS has issued advice in its DBS newsletter.
